申請聘僱外國專業人員工作許可應備文件（A類專門性或技術性工作-在臺畢業僑外生配額評點制）

2.於填寫或登錄繳費收據資料後，得予免附，惟必要時本部仍得視個案情形，請雇主檢附。(依勞動部111年11月25日勞動發事字第1110523178號公告)

4 評點表及符合各評點項目應備文件 Ｖ Ｖ 評點項目應備文件如附表。

曾獲評點制許可之外國人，如無欲加計點數之項目，得僅附評點表。
5 受聘僱外國人之護照影本或外僑居留證影本 Ｖ Ｖ 於聘僱起始日時仍應有效。
6 受聘僱外國人在臺取得學士學位以上之學歷畢業證書影本 Ｖ 　 　
7 聘僱契約書影本或副本 Ｖ Ｖ 應載明受聘僱外國人姓名、國籍、職稱或工作內容（應足資確認外國人實際從事之工作範疇）、薪資報酬、聘僱期間及經雙方簽章
8 申請單位負責人或建築師、會計師、律師(雇主)之國民身分證、護照影本或外僑居留證影本。 Ｖ 　 於申請日時仍應有效
9 申請單位立案登記證明影本或設立登記證明影本 Ｖ 　 自然人雇主免附
10 申請單位特許事業許可證等影本 Ｖ 　 依A類各申請工作項目別，如為應加附者請檢附。
11 申請單位營業額證明影本 Ｖ Ｖ 依A類各申請工作項目別，如為應加附者請檢附。
12 受聘僱外國人最近年度薪資扣繳憑單影本(含就源扣繳憑證影本) 　 （Ｖ） 得免附，惟必要時本部仍得視個案情形，請雇主檢附。(依勞動部111年11月25日勞動發事字第1110523178號公告)
13 原聘僱許可函影本 　 Ｖ 於填寫聘僱許可函文號後，得免附。(依勞動部111年11月25日勞動發事字第1110523178號公告)
14 受聘僱外國人未成年者，其法定代理人同意其聘僱之文件及法定代理人護照影本 Ｖ 　 　

※辦理終止聘僱關係（解聘）應備文件：1.申請書。2.聘僱許可函影本(於填寫聘僱許可函文號後，得免附)。3.居留證影本。4.聘僱關係終止證明文件。

※申請補發聘僱許可應檢附文件：1.申請書(請勾選申請書之「補發」選項，並寫欲補發之「工作許可」函號)。2.補發事由切結書。

※所附相關文件係英文以外其他外文者，應檢附中文譯本。

2.Need not attached once entering the data of receipt on the system (or filling in the application form).Nevertheless, it might be requested if necessary.

Foreigners having received comment permit in the past not adding score items only need attach comment form.
5 Photocopy of the passport of the employed foreign worker or ARC Ｖ Ｖ Must be valid on the date the employment begins. 
6 Photocopy of bachelor's degree or higher of the employed foreign worker obtained in the R.O.C. Ｖ 　 　
7 Copy or photocopy of the employment contract Ｖ Ｖ It must specify the name of the employed foreigner, nationality, job title or work
description (sufficient to tell the scope of work), employment period, wages and
compensation, and must be executed by both the employer and the employee.
8 Taiwanese ID, foreign passport or ARC of the person in charge of the entity or architect (employer)/ accountant (employer)/ lawyer (employer) Ｖ 　 Must be valid on the date of applying.
9 Photocopy of applicant entity’s company registration or business registration Ｖ 　 Exempt in the case where the employer is a natural person.
10 Photocopy of license for specially permitted business Ｖ 　 According to the items of Category A jobs, please confirm additional attachments.
11 Photocopy of applicant revenue certification Ｖ Ｖ According to the items of Category A jobs, please confirm additional attachments.
12 Photocopy of last year’s most recent payroll withholding statement of employed foreign worker (or tax withheld at source) 　 (Ｖ) Need not attached .Nevertheless, it might be requested if necessary.
13 Photocopy of original employment permit 　 Ｖ Need not attached once entering the permit number on the system.
14 When employed foreign worker is a minor, legal representative employment consent documents and photocopy of passport of legal representative Ｖ 　 　

※Documents required for termination of employment relationship (dismissal): 1. Application 2. Photocopy of employment permit(Need not attached once entering the permit number on the system). 3. Photocopy of residence permit 4. Document certifying termination of employment relationship

※Documents required for replacement employment permit: 1. Application (please check“Re-issue on application and write“permit number”) 2. Grounds for replacement affidavit.

※Any documents in a language other than English must be translated into Chinese.
